What It Is

Relacorilant is a selective glucocorticoid receptor antagonist. It doesn’t lower cortisol production in the body. Instead, it blocks cortisol from binding to its receptors in tissues, especially bone.

How it works

Cortisol is highly catabolic to bone. It suppresses osteoblasts and speeds up bone breakdown. Relacorilant binds to the glucocorticoid receptor and prevents cortisol from doing damage. This creates a more anabolic environment for bone tissue without directly raising GH or testosterone.

Benefits

• Reduces the negative effects of cortisol on bone.

• May support better bone density and periosteal growth (thicker bones, wider frame).

• Provides cleaner cortisol blockade with less interference on other hormone systems.

Key difference from older cortisol tools

Older options often hit multiple receptors and cause more hormonal disruption. Relacorilant is designed to be more selective for the glucocorticoid receptor only. This makes it theoretically better for protecting bone while keeping natural testosterone and estrogen balance more stable.

Doses

Start at 100 mg and only go up if tolerated well.

Keep cycles short: 6–12 weeks on, then equal time off.

Taken with food to reduce stomach discomfort.

Side Effects

• Generally milder than older cortisol blockers.

• Possible fatigue or mild mood changes.

• Can cause shifts in phosphate levels (hyperphosphatemia).

• Very limited research/data.
