i was thinking it was skin barrier issue because the products he uses are causing irritation and breaking out is also a causation of a bad barrier
Nah he says anything he eats breaks him out horribly, he just needs to take isotretinoin and the other stuff i proposed, but let me expand on that.

Isotretioin: dosed ED at 10-20mg (systemically stop oil production, improved cell turnover, reduces inflammation etc)

Tretinoin/Tazarotene: Only introduced if tolerated.

Topical E3: Used EOD or ED, works by binding to the ER and activating TGF beta signaling (Signals fibroblasts to increase synthesis of elastin and collagen type 1 and 3, and will deposit more hyaluronic acid and better absorption and will thicken the dermis)...

ok so your skin barrier is most likely fucked

i would use a low ph cleanser only at night, with a milk toner, hyaluronic acid, and moisturizer

cut out any actives

You only really need isotretinoin tbh, i don't see the need for all the other stuff. Sure it's good but keeping it minimal mogs

Look just get topical ivermectin and isotret, if i was you id still get topical E3 and tazarotene, just use as tolerated. You need ivermectin to get rid of the pustules and bumps and all that inflammation, it mogs azelaic acid and anthing else for that purpuose.

how, active will just make it worse
Use as tolerated, ivermectin is perfect to get rid of the bumps, inflammation, and vaso constrictions, which are what cause red bumps, the rest is just for the sake of improving and recovering the damaged collagen and elastin as topical E3 is super potent as it will activate all estrogen receptors in the skin and increase collagen and elastin synthesis, it will thicken the dermis and pull more hyaluronic acid into the tissue and therefore improve the hydration.

Same for tazarotene/tretinoin and isotret. He just needs to use them as tolerated and that's it. I would also add dutasteride too but idk if OP is 17+.

Not really, if anything it will make isotret work even better, again, use as tolerated.
