Isotretioin: dosed ED at 10-20mg (systemically stop oil production, improved cell turnover, reduces inflammation etc)
Tretinoin/Tazarotene: Only introduced if tolerated.
Topical E3: Used EOD or ED, works by binding to the ER and activating TGF beta signaling (Signals fibroblasts to increase synthesis of elastin and collagen type 1 and 3, and will deposit more hyaluronic acid and better absorption and will thicken the dermis).
Ivermectin topical: EOD To get rid of the red bumps and pustules caused by inflammation and vasoconstrictions.
A non comedogenic moisturizer (OPs choice).
I would introduce dutasteride EOD or ED depending if op is older than 17+ to further avoid the collagen rape from DHT.

Ivermectin tablets?Look just get topical ivermectin and isotret, if i was you id still get topical E3 and tazarotene, just use as tolerated. You need ivermectin to get rid of the pustules and bumps and all that inflammation, it mogs azelaic acid and anthing else for that purpuose.
